Brake pressure loss

My buddy is experiencing brake pressure loss in his car any ideas if the prob.

Start with the obvious:

1-Brake fluid been flushed recently ? Or EVER ? If so, the right fluid used ?
2-Chk the pads
3-Chk the rotors
4-Chk the brake lines for crimps/worn areas/lines
5-Brake booster
